In accordance with State of Texas requirements, the information below is provided to guide our clients in making a report, should that become necessary.

We are required to retain your records for 7 years, or 5 years after a former child client has turned 18. If needed, records can be requested thru your patient portal or by email at info@unleashinghope.family.

This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service.
