Assessments

Unleashing Hope Counseling, PLLC offers several assessments as a separate service for those clients wanting information about their baseline starting point. We also provide other assessments and screenings for existing clients. 

CANS

Child & Adolescent Needs and Strengths

The CANS is a multi-purpose assessment tool developed to outline areas of strengths and needs that can be utilized in developing a strong treatment and support plan. It can help determine whether a need has been resolved and strengths created. It can also be used to celebrate the successes of a child. 


 This assessment can be included as an existing client; as well as provided to one-time assessment only clients who wish to take the report to an existing mental health professional.  


*The CANS may be covered by insurance depending on the date of your last assessment. 

DLA-20

Daily Living Activities

The DLA-20 is a functional assessment that can help clinicians justify medical necessity, measure progress or lack of progress, and focus on specific areas of improvement.  It identifies areas of strengths, weaknesses, and impairments in functioning; as well as the severity of issues. This outcome tool also helps with the creation of a treatment plan. 


The youth version is for youth ages 6-17 and requires the participation of a parent/caregiver in the assessment session. The adult version is for adults 18 and up. If the adult struggles with intellectual disabilities or cognitive impairments, a parent/caregiver will need to attend in order to provide accurate information of functioning. 


This assessment can be included as an existing client; as well as provided to one-time assessment only clients who wish to take the report to an existing mental health professional.
